We will use the information that you provide in this Privacy Rights Request Form to process your Privacy Rights Request and to maintain a record of your Privacy Rights Request and our response.

Proper authentication is critical to protect your personal information. You may be required to provide proof of identity before your request is fulfilled. We will respond consistent with applicable laws. For all submissions, we will respond to your request within 10 business days and complete your request within 45 days. If your request requires more time to complete, we will contact you with an explanation and next steps.
